Community nursing

Community (District) Nurses provide a service for people who are housebound: that is when a person is unable to leave their home due to a physical and/or psychological illness; or where there is a nursing need which makes a home visit more appropriate.

They work with patients and their families/carers to help identify your needs and find solutions.  This can include:

  • general assessment and care planning
  • leg ulcer and wound care management
  • continence assessment including catheter and stoma care
  • diagnostic tests - blood pressure, blood and urine tests
  • immunisation and vaccination
  • palliative and end of life care
  • teaching and support

Please note that the District Nursing Service does not attend where there is no nursing need, and is unable to provide routine help with personal hygiene, respite meals, housework, oral medicines or collecting prescriptions.
